How Rewards Work

  • Earn Rate
    10–15% of your net order value (excl. shipping/tax/discounts) paid in FORM.
  • Claim Timing
    Rewards are released after the return window closes (default: 14 days) to prevent abuse.
  • Payout Method
    Claim from your BlockForm Account → FORM Rewards.
  • Wallet
    Add an EVM wallet address in your account to withdraw.
  • Tiers
    Standard: 10% VIP: 12.5% (lifetime spend ≥ $500 or 3 orders in 90 days) Elite: 15% (lifetime spend ≥ $2,000 or invite-only)
  • Caps & Minimums (recommended)
    Min claim: 250 FORM (batch small rewards) Max per order: 200,000 FORM (anti-abuse) Excluded items: Gift cards, final-sale, or heavily discounted items can be excluded.

Claim & Withdraw

  • Create/Sign in to your BlockForm account.
  • Add your Base wallet address (Coinbase Wallet, MetaMask, etc.).
  • After your return window closes, your reward appears in Account → FORM Rewards.
  • Click Withdraw to send to your wallet (gas fees apply).

Tip: Keep FORM in your account to unlock members-only drops and early access.
